As Aging Life Care Managers – or Geriatric Care Managers – our goal is to support seniors and their families in working through the medical, financial and legal challenges that often come with growing older. Whether you have a loved one who you want to make sure is safe in their home or you’re faced with a chronic medical issue, such as memory loss, and need assistance in dealing with it, we can help. Our compassionate care managers have extensive experience in providing solutions to aging’s most challenging problems.

Here are just some of the services we provide:

Assessment and Evaluation

Our first step is to meet the individual and family members to determine what’s needed.

  • Assess the individual’s home environment and family/support systems to determine current physical, emotional, social, financial, and safety needs and determine what needs may arise in the future.
  • Develop an individualize Care Plan family that provides resources and solutions for both immediate and long-term.

Coordination and Implementation

Once a Care Plan is in place, we help manage and implement the plan, which may include:

  • Recommending and helping coordinate the services that will help achieve an individual’s goals – including physicians, home health care, financial planners, elder law specialists, rehabilitation therapists, private nursing services, senior living communities and hospice.
  • Coordinating Medicare, Medicaid and private pay services.
  • Providing assistance in creating an Advance Directive and end-of-life care planning.
  • Recommending and coordinating home safety modifications such as ramps, grab rails, shower access and door widening.
  • Recommending and assisting with moves to senior living communities or skilled nursing/rehabilitation facilities.
  • Providing referrals for transportation, meal delivery, financial management and other services.
  • Managing medical crises
  • Educating individuals and families

Counseling and mediation

Often, both the elder and their families need assistance in dealing with the issues of aging. We can provide:

  • Individual and family counseling to deal with issues of depression, grief, and interpersonal issues. This can be done in the home for your convenience.

Mediation to resolve conflicts that arise among family members or the individual and the caregiver, facility or care professional.
